Dear Members of Congress,

We are delighted to present the 2nd International Forum on Clinical and Hospital Pharmacy (FIFAR2025). The event will focus on the most important and up-to-date topics in these areas, with the central theme being "Pharmaceutical Care and the Transition of Care between Hospitals, Reference Centres and Primary Health Care". The event will provide an opportunity for debate with the main players on the national scene in different aspects of the clinical area.

Pharmaceutical education with a focus on clinical pharmacy and pharmaceutical care represents a major challenge for the universities and pharmacy courses available in Brazil. The parallel between pharmaceutical education to start a debate on the prospects of the profession in a scenario of a universal health system and within a network of university hospitals under the management of EBSERH.

Furthermore, and no less importantly, it is necessary to qualify pharmaceutical professionals to work in teams, in an interdisciplinary and multi-professional manner.

This event aims to debate some important issues in the context of the SUS, such as: Implementation of the Ministry of Health's Clinical Protocols and Therapeutic Guidelines in an articulated way between primary and tertiary care; Can pharmaceutical care be included in health care? How can pharmaceutical services be reorganised to promote the inclusion of clinical pharmacists in healthcare teams? How can teaching and research be developed in the area of pharmaceutical care, taking into account the pharmacotherapeutic needs of patients?
